Seems to be working now. I think that happens when the story gets approved by a mod but then the author makes an edit afterwards. It’ll still appear on the front page sometimes but be inaccessible until it’s approved again.
Or I might not know what the hell I’m talking about.

What Swizzington said, but let me add, that in some cases forcing a browser refresh (reloading without using the cache) can help as well.
In Chrome, this can be done simply by pressing Strg-F5.
In mobile browsers, however, the only way to force this is to delete the local browser cache.
Always try this first in case you have some problem.
